HOW TO COMPLETE THIS ASSESSMENT:

The assessment is divided into six categories and for each one you will evaluate four related statements.

You will have 200 points available to divide across each set of statements:

100 points to describe culture as it is now
100 points to describe culture in your ideal future

You can assign as many or as few points as you want, as long as each column adds up to 100 (Now-100, Future-100). The more points you assign, the more you agree with the statement and vice versa.

Once you’ve rated each set of statements, you will have the opportunity to share additional feedback and insights into department culture.

The survey is anonymous, and all results will be aggregated by team and/or department before being shared.
